Why Could Nifedipine Affect the Heart?
Calcium channels are involved in the electrical and mechanical activity of heart cells.
The researchers performed laboratory experiments involving human-induced pluripotent stem-cell-derived cardiomyocytes. They reported differences in the effects of nifedipine and amlodipine on cardiac electrical activity.
At clinically relevant concentrations, nifedipine produced greater L-type calcium-current blockade and greater shortening of the cardiac action potential than amlodipine. (OUP Academic)
This provides a possible biological explanation for the epidemiological findings.
But laboratory findings do not by themselves prove that a particular patient taking nifedipine will develop a dangerous rhythm.
They help researchers investigate how an observed association might occur.
There Is Another Important Nifedipine Safety Issue
The history of nifedipine contains another important lesson.
Short-acting or immediate-release nifedipine should not be confused with modern long-acting formulations.
Rapidly lowering blood pressure can sometimes reduce blood flow to vital organs.
Historically, immediate-release nifedipine was sometimes used to rapidly lower very high blood pressure. This approach fell out of favor because uncontrolled, rapid reductions in blood pressure can potentially cause serious complications, including inadequate blood flow to the heart and brain.
Current clinical references specifically caution against using immediate-release nifedipine in hypertensive emergencies or urgencies in this manner.
This is very different from taking a prescribed long-acting nifedipine formulation for ongoing blood-pressure control.
Does This Mean Nifedipine Should No Longer Be Used?
No.
That conclusion would not accurately reflect current clinical guidance.
The 2024 European Society of Cardiology hypertension guidelines continue to list dihydropyridine calcium-channel blockers among the major first-line medication classes for hypertension.
The 2025 American Heart Association/American College of Cardiology hypertension guideline similarly recommends long-acting dihydropyridine calcium-channel blockers as one of the first-line options for adults beginning antihypertensive treatment.
In other words, the existence of research questioning the safety profile of a particular drug or dose does not automatically mean the entire medication class has been abandoned.
Why Treating High Blood Pressure Still Matters
There is another side to the story that viral posts can easily overlook.
Untreated or poorly controlled hypertension itself is a major cardiovascular risk factor.
Research involving millions of people has found a strong association between hypertension and sudden cardiac arrest. One large population-based study involving more than four million people found that people with hypertension had substantially higher risk of sudden cardiac arrest than those without hypertension.
Another large study found that increases in systolic blood pressure over time were associated with higher sudden-cardiac-arrest risk.
Therefore, the goal is not simply:
“Avoid blood-pressure medication.”
The goal is:
Control blood pressure safely with the medication and treatment strategy that is appropriate for the individual patient.
